The South East London Integrated Care Board has renewed its contract for the Digital Health Passport (DHP) through to 2027, extending the programme beyond asthma to include epilepsy care.
The renewal reflects the ICB's continued commitment to providing practical, trusted digital support for children and young people living with long-term conditions — and to building on what has already been shown to work locally.
Developed by Tiny Medical Apps, the Digital Health Passport is an NHS-supported mobile app designed to help people better understand their condition, follow personalised care plans, and manage day-to-day health with confidence.
Supporting asthma and allergy care across South East London
The Digital Health Passport is already available locally to support people living with asthma and allergies, with a particular focus on children and young people, as well as their parents and carers.
The app is co-designed with young people and clinicians, ensuring it reflects real lives rather than clinical theory alone. It supports self-management alongside routine NHS care — not as a replacement for appointments, but as a day-to-day companion.
