We are delighted to announce that the Digital Health Passport is now being rolled out in Staffordshire & Stoke-on-Trent ICB

The Staffordshire and Stoke-on-Trent Integrated Care Board (ICB) has made a significant stride in asthma self-management with the successful launch of the Digital Health Passport. Developed by Tiny Medical Apps, this app aims to empower children, young people, and their caregivers in effectively managing asthma while providing access to a wealth of educational resources to boost their understanding and confidence in handling this condition.

The Digital Health Passport offers a suite of features specifically designed to cater to the unique needs of asthma patients. One of its standout functionalities is the ability to share asthma management plans with family and friends, fostering a collaborative approach to support individuals dealing with asthma. Users can easily monitor their symptoms and access educational resources, enabling them to make well-informed decisions about their health.

Additionally, the app promotes connectivity within the asthma community by providing a platform for peer support and access to online resources from trusted partners like Asthma & Lung UK. Practical tools such as medication reminders and advice on avoiding triggers greatly enhance the overall asthma management experience.

In an exciting development, Staffordshire and Stoke-on-Trent are one of the ICBs collaborating with Tiny Medical Apps to evaluate the impact of materials like posters and flyers containing a QR code to download the app, within their Community and Secondary Care setting. This initiative aims to increase patient adoption and activation.
