The Digital Health Passport – a personal health record app for teenagers with asthma is now live at 3 sites in east London.
The app has a digital version of an Asthma Action Plan – recommended by NICE – and enables young people to track symptoms, access educational resources and find local services, as well as receive air quality alerts. It has been commissioned by the NHS Healthy London Partnership as part of a drive to improve asthma outcomes in the capital.
The Digital Health Passport has been designed for teenagers to take control of their health – creating asthma action plans, tracking symptoms and accessing NHS support.
The project has been led and commissioned by the NHS Healthy London Partnership, Children & Young People’s team. Selected by NHS Digital to be amongst the first to support NHS Login.

Where has it been launched?
We are live in Barts Health and Chrisp Street Health Centre in east London.
Can it be commissioned in my area?
We are keen to talk to commissioners of young people’s health services across the country. Our product is on this years ITP Evidence Generation Fund: Interoperable Personal Health Record tool.
